In order for your child to attend public Kindergarten, he/she must meet the requirements set jointly by the Massachusetts Department of Public Health and the Foxborough Public School Department.
No child will be permitted to start school in September without documentation of the following Kindergarten requirements:
- A physical exam completed within l year prior to school entry
- Proof of lead screening
- A complete immunization record
- -D.P.T. (diphtheria, tetanus, pertussis) 5 doses
- -OPV/IPV (polio vaccine) 4 doses
- -M.M.R. (measles, mumps, rubella) 2 doses
- -Hepatitis B vaccine 3 doses
- -Hib Vaccine 3 or 4 doses
- -Varicella – 2 doses of the vaccine OR physician certified history of chicken pox disease, please list month and year.
- Certification that the child has passed a vision screening within the previous 12 months. *This protocol requires visual acuity testing for both eyes as well as stereopsis testing.
Please submit required physical exam and immunization documentation as soon as you have it but NO LATER than the first day of school (September 2021). FAILURE TO DO SO MAY RESULT IN EXCLUSION FROM SCHOOL. Please direct attention to school nurse on envelope if mailing or hand deliver to school nurse if possible to avoid misplacement. For your convenience, your doctor may fax the records directly to the nurse’s office.
